Methandrostenolone (trade name: Dianabol; informal: D-bol) was one of the first synthetic anabolic steroids developed specifically for anabolic use without primary androgenic intent. Synthesized by Ciba Pharmaceuticals (John Ziegler, 1955–1958) and approved by the FDA in 1958 for muscle-wasting conditions, it became the most widely used oral anabolic steroid in medical and research history. Structurally, it is a C17α-methylated Δ1-testosterone (with additional aromatization capability), making it the oral analogue of boldenone with potent anabolic activity but significant estrogenic side effect potential.

Structural Relationship to Testosterone and Boldenone#
Methandrostenolone = 17α-methyl-Δ1,4-androstadien-17β-ol-3-one:
- Testosterone + Δ1,2 double bond → Δ1-testosterone (= boldenone with full oral form modification)
-
- C17α-methyl group → oral bioavailability
Comparison:
| Feature | Testosterone | Boldenone | Methandrostenolone |
|---|---|---|---|
| Δ1,2 double bond | No | Yes | Yes |
| C17α modification | No | Undecylenate ester (injectable) | Methyl (oral) |
| Aromatization | High | ~50% T | ~50% T (similar to boldenone) |
| 5α-reduction | DHT (potent) | Δ1-DHT (moderate) | Δ1-DHT (moderate) |
| Route | IM injection | IM injection | Oral |
Methandrostenolone is essentially oral boldenone — the Δ1,2-double bond reduces aromatization by ~50% vs. testosterone and slows 5α-reduction, similar to boldenone, but the C17α-methyl group enables oral use (at the cost of hepatotoxicity).
Pharmacokinetics: Oral t½ and Dosing#
| Parameter | Value |
|---|---|
| Oral bioavailability | ~60–70% (C17α-methyl enables oral use) |
| t½ | 3–6 hours |
| Dosing interval | 2–3× per day for stable levels |
| Metabolism | Hepatic CYP3A4; 17-epimethandrostenolone (major metabolite) |
| Urinary detection | Up to 19 days (WADA — 17-epimethandrostenolone metabolite) |
Rodent PK:
- Faster metabolism in rats; t½ ~1–2h
- Typically administered by oral gavage 2–3× daily or in food/drinking water
Aromatization and Estrogenic Activity#
Methandrostenolone aromatizes to 17α-methylestradiol (not standard estradiol):
- Aromatase converts methandrostenolone → 17α-methylestradiol (a potent, unusual estrogen)
- 17α-methylestradiol has HIGHER ERα binding affinity than estradiol-17β (E2) in some studies
- Result: despite ~50% lower aromatization rate vs. testosterone per molar dose, methandrostenolone's estrogenic metabolite may be more potent per mole → net estrogenic activity comparable to or exceeding testosterone at equivalent anabolic doses
Research implication: methandrostenolone is NOT a useful compound for studying pure AR effects — the estrogenic confound from 17α-methylestradiol is significant and difficult to block with standard aromatase inhibitors (which may be less effective against methylestradiol formation than standard E2).
For pure AR studies, use a non-aromatizing compound: stanozolol, oxandrolone, trenbolone, or DHT.
AR Binding and Anabolic Signaling#
Methandrostenolone AR binding:
- Relative AR binding affinity: ~1.2–1.5× testosterone (moderate enhancement vs. testosterone, similar to nandrolone)
- Direct AR agonist (does not require 5α-reduction; already has Δ1,2 modification reducing 5α-reductase activity)
- ARE-driven anabolic gene expression: IGF-1, follistatin, mTOR pathway components
Compared to boldenone at the AR: Essentially equivalent — methandrostenolone and boldenone have similar AR binding affinity and similar ARE-driven transcriptional activity (both are Δ1-testosterone derivatives). The C17α-methyl doesn't significantly change AR binding geometry.
A:A Ratio and Clinical Anabolic Effects#
A:A ratio: ~1.6:1 (levator ani / ventral prostate, Hershberger assay)
Moderate anabolic selectivity — less than nandrolone (~3:1) but more than testosterone (~1:1). The high absolute anabolic potency means effective muscle building at lower androgenic burden, but the estrogenic metabolite (17α-methylestradiol) adds a third pharmacological dimension not captured by the Hershberger A:A ratio.
Historical clinical anabolic effects:
- Methandrostenolone 5–10 mg/day in hypogonadal men or muscle-wasting patients → significant lean mass gains (2–4 kg in 6 weeks in early clinical studies)
- Nitrogen balance studies: positive nitrogen retention at 5 mg/day (early 1960s clinical pharmacology)
- Peak plasma nitrogen retention: ~4g/day positive nitrogen balance at 15 mg/day
- These historical data established the principle that oral anabolic steroids could meaningfully affect protein metabolism
Historical Research Applications#
Methandrostenolone has the longest research history of any oral anabolic steroid:
Muscle protein synthesis (1960s–1980s)#
- Nitrogen balance studies: early evidence that androgens could produce positive nitrogen balance (more nitrogen retained in tissue than excreted)
- 14C-leucine incorporation studies: ¹⁴C-labeled leucine → methandrostenolone → increased ¹⁴C incorporation into muscle protein vs. control
- These studies established fundamental principles of androgen-driven anabolism
Osteoporosis (1970s–1980s)#
- Methandrostenolone prescribed for postmenopausal osteoporosis (before bisphosphonates)
- Evidence: increased bone mineral density on DXA-equivalent measurements
- Discontinued for this indication due to virilization in women and hepatotoxicity
HIV/AIDS wasting and cancer cachexia (research model)#
- Methandrostenolone as oral agent for wasting: active AR agonism → muscle protein preservation
- Less used than oxandrolone for modern wasting research due to estrogenic confound
- But as historical comparator in older literature, important reference compound
