PT-141 (Bremelanotide) is a cyclic heptapeptide melanocortin receptor agonist (MC3R and MC4R) derived from Melanotan II, acting through central nervous system pathways rather than the peripheral vascular mechanism of PDE5 inhibitors. By activating dopaminergic and cholinergic pathways in the paraventricular nucleus of the hypothalamus, it initiates sexual arousal signals independent of nitric oxide. The FDA approved Bremelanotide (Vyleesi) in 2019 for premenopausal women with hypoactive sexual desire disorder — the only approved melanocortin-targeting compound for sexual health.
