Sildenafil (trade name: Viagra; Pfizer, 1998) was the first oral PDE5 inhibitor approved for erectile dysfunction, and it remains the most studied PDE5 inhibitor in the scientific literature. Originally developed as a cardiovascular compound targeting PDE5-mediated cGMP in vascular smooth muscle, sildenafil's ED application emerged serendipitously during clinical trials. With over 25 years of research data, sildenafil is the reference PDE5 inhibitor for in vitro enzyme assays, cell-based cGMP studies, and animal models of erectile and vascular function.

PDE5 Enzyme: Target Biology#
PDE5 (phosphodiesterase type 5):
- Class: cyclic nucleotide phosphodiesterase (hydrolyzes 3',5' phosphodiester bond)
- Substrate specificity: cGMP (>>> cAMP; Ki cGMP ~100-fold lower than cAMP)
- Expression: corpus cavernosum (highest), pulmonary vascular smooth muscle, platelets, GI smooth muscle, kidney, brain (low)
Regulation:
- PDE5 activity regulated by cGMP itself: cGMP-binding allosteric site (GAF-A domain) → cGMP binding → enzyme activation (auto-sensitization)
- cGMP/PKG → phosphorylates PDE5 → increases V_max → faster cGMP degradation (negative feedback loop)
- Consequence: elevated cGMP degrades itself faster — sildenafil breaks this loop
Why PDE5 inhibition amplifies the NO signal: Without sildenafil: NO → cGMP → partial PKG activation → PDE5 degrades cGMP back to baseline quickly With sildenafil: NO → cGMP → partial PKG activation → PDE5 blocked → cGMP accumulates → sustained PKG → maximal smooth muscle relaxation
Sildenafil Mechanism and PDE Selectivity#
Sildenafil binding to PDE5:
- Competitive with cGMP at the catalytic site (not the allosteric GAF site)
- IC50 for PDE5 hydrolysis: ~3–5 nM
- Selectivity profile vs. other PDEs:
| PDE | Sildenafil IC50 | Consequence |
|---|---|---|
| PDE5 | ~3–5 nM | Target (ED, PAH) |
| PDE6 | ~30–100 nM | Retinal phosphodiesterase → visual disturbances (blue tinge, photosensitivity) — most unique sildenafil side effect |
| PDE1 | ~200 nM | Heart, brain — minimal |
| PDE2, 3, 4 | >1000 nM | Minimal |
| PDE11 | >1000 nM | Minimal (vs. tadalafil ~37 nM) |
PDE6 crossover is sildenafil's most distinctive off-target effect — it causes the visual side effects (blue/green tinge, photophobia, blurred vision) reported in early clinical trials and persists at therapeutic doses. In research, this means sildenafil should NOT be used in retinal or photoreceptor research where PDE6 inhibition would confound results.
Pharmacokinetics#
| Parameter | Sildenafil | Tadalafil |
|---|---|---|
| Oral bioavailability | ~40% (high-fat meal reduces further) | ~80% (food-independent) |
| t½ | ~4 hours | ~17.5 hours |
| Tmax | 0.5–2 hours (faster onset) | 2–4 hours |
| Active metabolite | N-desmethylsildenafil (~50% PDE5 potency, t½ ~4h) | Catechol metabolite (inactive) |
| Protein binding | ~96% | ~94% |
| Metabolism | CYP3A4 (primary), CYP2C9 | CYP3A4 |
Rodent PK (rat):
- t½ ~1–2h (faster)
- For acute protocols: dose 30–60 min before measurement
- For chronic protocols: twice-daily dosing maintains coverage
ED Research Models#
Cavernous nerve stimulation model (primary ED research model)#
| Parameter | Specification |
|---|---|
| Preparation | Adult male rat, urethane anesthesia (1.2 g/kg IP) |
| Surgical setup | Abdominal incision; cavernous nerve (CN) identified bilaterally; bipolar platinum electrodes placed on CN |
| Intracavernous pressure | Butterfly needle (25G) inserted in corpus cavernosum → connected to pressure transducer |
| Stimulation | Electrical CN stimulation (3–5V, 1–5 Hz, 0.2 ms pulse width, 60s) |
| Sildenafil administration | IV bolus (0.1–1 mg/kg) or oral pre-treatment 30 min prior |
| Primary endpoint | ICP rise (mmHg) / mean arterial pressure (MAP) ratio — ICP/MAP ratio ≥ 0.6 = normal erectile response |
Diabetic ED model (streptozotocin rats)#
| Parameter | Specification |
|---|---|
| Induction | STZ 60 mg/kg IV → type 1 diabetes (verify by blood glucose >300 mg/dL) |
| Duration before testing | 8 weeks (established ED) |
| Treatment | Sildenafil 0.5 mg/kg orally daily × 4 weeks; vehicle control |
| Endpoints | ICP/MAP during CN stimulation, penile eNOS expression (Western blot), cGMP content (ELISA), PDE5 protein levels |
Cardiovascular and PAH Research#
Sildenafil is FDA-approved for PAH as Revatio (20 mg three times daily):
- Pulmonary vascular smooth muscle: high PDE5 expression → sildenafil → pulmonary vasodilation
- Mechanism identical to ED: NO/cGMP preserved → pulmonary arterial smooth muscle relaxes
Advantages of sildenafil over tadalafil in acute PAH research:
- Faster onset → better for acute hemodynamic studies
- Shorter t½ → clean washout for crossover designs
- Most extensively published reference compound
Sildenafil vs. Tadalafil: Research Comparison#
| Research application | Preferred compound | Reason |
|---|---|---|
| Acute ED/cavernous nerve model | Sildenafil | Faster onset, better IV formulation |
| Chronic daily ED model | Tadalafil | Long t½, once-daily dosing |
| Acute PAH hemodynamics | Sildenafil | Rapid onset |
| Chronic PAH remodeling | Either | Both validated; tadalafil once-daily convenient |
| PDE5 biochemical assay (reference) | Sildenafil | Most published IC50 data |
| Where PDE6 confound matters (retina) | Tadalafil | Better PDE6 selectivity |
| Where PDE11 confound matters (testes) | Sildenafil | Better PDE11 selectivity |
| BPH research | Tadalafil | Approved indication; better bladder/prostate data |
