Clomiphene citrate (trade name: Clomid; Serle/Sanofi) is a SERM approved clinically for ovulation induction in anovulatory women. It is a racemic mixture of two geometric isomers with opposite ER pharmacology — enclomiphene (trans-isomer: ER antagonist) and zuclomiphene (cis-isomer: ER partial agonist). The ER antagonist component (enclomiphene) drives its clinical and research utility: antagonism at hypothalamic/pituitary ERα removes E2 negative feedback, elevating GnRH, LH, FSH, and ultimately endogenous testosterone. In androgen research, clomiphene is primarily used as a tool to stimulate the HPTA while maintaining endogenous testosterone production.

Isomer Pharmacology: Enclomiphene vs. Zuclomiphene#
Clomiphene citrate = 50:50 racemic mixture:
| Property | Enclomiphene (trans-) | Zuclomiphene (cis-) |
|---|---|---|
| ER activity | Antagonist (dominant pharmacological effect) | Partial agonist |
| t½ | ~24 hours | ~30 days (accumulates with chronic dosing) |
| HPTA effect | Stimulates LH/FSH | Partially blunts LH/FSH response |
| Proportion in clomiphene citrate | 50% | 50% |
| Visual side effects | Less | More (believed primary cause of blurred vision) |
Key insight: the enclomiphene component is largely responsible for HPTA stimulation. Zuclomiphene's long t½ means it accumulates with chronic dosing and partially antagonizes enclomiphene's effects — this is why isolated enclomiphene (without zuclomiphene) has been developed as a purer HPTA stimulant.
Mechanism of HPTA Stimulation#
In the context of estrogen-suppressed or hypogonadal males:
- Clomiphene (enclomiphene component) → binds hypothalamic ERα
- Blocks E2 negative feedback on GnRH neurons → increased GnRH pulse frequency and amplitude
- Pituitary gonadotrophs: ERα block → removes E2 suppression of LH/FSH synthesis and release
- LH surge → Leydig cells → increased testosterone synthesis (cAMP → StAR → CYP11A1 → CYP17A1)
- FSH → Sertoli cells → spermatogenesis support
- Net: increased testosterone + preserved/improved spermatogenesis
Compared to exogenous testosterone replacement:
- Exogenous TE: high serum testosterone, suppressed LH/FSH, suppressed spermatogenesis, testicular atrophy
- Clomiphene: modest testosterone increase (endogenous, limited by Leydig cell capacity), preserved LH/FSH, preserved/improved spermatogenesis
This makes clomiphene particularly valuable in fertility research — it maintains the full HPT axis while increasing testosterone.
Pharmacokinetics: Isomer Differences#
| Parameter | Enclomiphene | Zuclomiphene |
|---|---|---|
| t½ | ~24–26 hours | ~30 days |
| Steady-state | ~5 days | ~3 months |
| Excretion | Hepatic metabolism → feces | Hepatic → feces, slow |
| Clinical implication | Washes out quickly after cessation | Persists; contributes to side effects |
Research implication: in a 4-week clomiphene study, zuclomiphene has not yet reached true steady state (requires months). Serum level measurements must specify which isomer is being measured, or use total clomiphene. Modern assays can separate isomers by chiral HPLC or LC-MS/MS.
Clomiphene in Male Hypogonadism Research#
Male hypogonadism research with clomiphene:
Secondary hypogonadism (intact HPTA, low LH/FSH/T):
- Clomiphene 25–50 mg every other day → LH +30–80%, FSH +30–80%, testosterone +50–100%
- Testicular volume maintained or increased
- Spermatogenesis parameters: improved in most subjects
Post-androgen suppression recovery:
- After testosterone replacement cessation: clomiphene accelerates HPTA recovery vs. spontaneous
- Combined with hCG: hCG stimulates Leydig cells directly while clomiphene restores HPTA top-down signal
- Clomiphene alone vs. tamoxifen alone: comparable LH/FSH elevation in most research; clomiphene has more visual side effect reports
Rodent model: clomiphene in hyperprolactinemia + hypogonadism
- Not effective for tertiary hypogonadism (hypothalamic GnRH deficiency) — clomiphene requires a responsive hypothalamus to work
Enclomiphene as a Distinct Drug#
Enclomiphene (trans-clomiphene) has been investigated as an isolated compound:
- Removes zuclomiphene (with its long t½ and partial agonism confound)
- Clinical trials: enclomiphene 12.5–25 mg/day → testosterone normalization + preserved spermatogenesis
- Cleaner PK (short t½, no accumulation) → easier research protocol management
- Currently in the SERM research toolkit as a more defined pharmacological tool than racemic clomiphene
In research comparing HPTA agents: enclomiphene provides a cleaner ER antagonist signal than clomiphene citrate and should be preferred when the goal is studying pure ERα antagonism at the hypothalamus.
Ovulation Induction (Female Models)#
Clomiphene is FDA-approved for ovulation induction:
- Mechanism: antagonizes hypothalamic/pituitary E2 negative feedback in females
- Result: elevated FSH → stimulates ovarian follicle development
- Applied in polycystic ovary syndrome (PCOS) research, anovulation models
Rodent model:
- Female rats: clomiphene 10 mg/kg days 1–5 → ovulation (vaginal smear confirmation) + follicle count by histology
- Comparison with letrozole (also used for ovulation induction) → mechanistically different (letrozole: reduces E2; clomiphene: blocks ER feedback)
