Exemestane (Aromasin) is a steroidal, irreversible aromatase inactivator — classified as a 'suicide inhibitor' because it permanently deactivates CYP19A1 aromatase by forming an irreversible covalent bond. Unlike non-steroidal AIs (anastrozole, letrozole) which are reversible competitors, exemestane's effect persists until new enzyme is synthesized, providing sustained estrogen suppression even between doses. Additionally exhibits weak androgenic properties due to its steroidal backbone, which partially mitigates AI-associated bone density losses.
