The combination of Ipamorelin (a GHS-R1a agonist) and CJC-1295 (a GHRH analog) represents the most studied and mechanistically rational approach to pituitary GH stimulation in research. The two compounds act on entirely different receptor systems, produce synergistic GH release when combined, and together provide more complete GH axis coverage than either compound alone.
This article covers the pharmacological basis for the combination, the synergism mechanism, published evidence, and protocol design for Ipamorelin + CJC-1295 research.

Two Different Receptors: Why the Combination Works#
The mechanistic foundation of the Ipamorelin + CJC-1295 combination is receptor complementarity:
-
Ipamorelin binds GHS-R1a (Growth Hormone Secretagogue Receptor type 1a) — the ghrelin receptor. This receptor is expressed on pituitary somatotrophs and partially suppresses somatostatin's inhibitory signal.
-
CJC-1295 binds GHRH-R (Growth Hormone-Releasing Hormone Receptor) — a completely separate receptor on pituitary somatotrophs. GHRH-R activation directly drives GH synthesis and release.
Because these receptors are distinct and activate different intracellular pathways, their effects are additive to synergistic rather than redundant. You are not double-dosing the same receptor — you are activating two independent GH-stimulating inputs simultaneously.
Analogy: pushing a car with two people pushing from different angles is more effective than two people at the same spot. The inputs converge on the same outcome (the car moves) without competing for the same contact point.
CJC-1295: GHRH Analog Pharmacology#
CJC-1295 is a synthetic analog of GHRH — the endogenous hypothalamic hormone that drives pituitary GH release. The full name is CJC-1295 DAC (drug affinity complex) or CJC-1295 without DAC (also called Modified GRF 1-29, or Mod-GRF).
Structure#
CJC-1295 is a 30-amino-acid GHRH analog (vs. 44 amino acids in full-length GHRH) with:
- Four amino acid substitutions from native GHRH to increase stability and receptor binding
- Optional DAC (Drug Affinity Complex): a lysine side-chain modification that allows the peptide to bind serum albumin reversibly → dramatically extends plasma half-life from ~30 minutes to 6–8 days
Receptor binding#
At GHRH-R, CJC-1295 is a full agonist — it activates the same intracellular pathway as endogenous GHRH (Gs → adenylate cyclase → cAMP → PKA → GH secretion and somatotroph proliferation).
Why not use GHRH directly?#
Native GHRH has a plasma half-life of ~7 minutes (rapidly cleaved by plasma dipeptidylpeptidase IV). CJC-1295 with the four substitutions resists DPP-IV cleavage, producing a half-life of ~30 minutes (without DAC) or 6–8 days (with DAC).
The Synergism Mechanism#
The synergism between Ipamorelin and CJC-1295 operates through three complementary mechanisms:
1. Dual receptor activation#
As described above, GHS-R1a (Ipamorelin) and GHRH-R (CJC-1295) activate different intracellular pathways in somatotrophs:
- GHRH-R → cAMP → PKA → GH gene transcription + immediate secretion
- GHS-R1a → Gq → PLC → IP3/DAG → calcium mobilization → rapid exocytosis
Both signals converge on GH granule release, but via different calcium mobilization mechanisms. The calcium signals from both pathways summate → greater GH exocytosis than either alone.
2. Somatostatin suppression#
Ipamorelin (via GHS-R1a at the hypothalamus) partially suppresses somatostatin release. Somatostatin is the inhibitory brake on GH pulsation. When Ipamorelin reduces somatostatin tone, CJC-1295's GHRH signal is less opposed → larger effective GH pulse.
This is the single most important synergism mechanism: CJC-1295 tells the pituitary to release GH; Ipamorelin lifts the somatostatin brake that would otherwise blunt the CJC-1295 signal.
3. Increased GH release per pulse#
The combination produces GH pulses with higher amplitude than either compound alone at equivalent doses. Studies in rats and humans show the combination produces 3–10× greater peak GH than monotherapy — a synergistic (greater than additive) effect in many conditions.
Published GH Release Data: Combination vs. Monotherapy#
Preclinical (rat) data#
Teichman et al. (2006) and multiple other groups have reported:
| Condition | Peak GH (arbitrary units) |
|---|---|
| Vehicle (control) | Baseline |
| CJC-1295 alone (100 µg/kg) | 3–5× baseline |
| Ipamorelin alone (100 µg/kg) | 4–6× baseline |
| CJC-1295 + Ipamorelin combined | 15–30× baseline |
The synergism is unmistakable: the combination produces 3–5× the GH release of either compound alone at the same doses. This is the primary published justification for the combined protocol.
Human data (CJC-1295 with DAC)#
Teichman et al. (2006) studied CJC-1295 with DAC in healthy adults:
- Single SC injection produced GH elevation lasting 9–11 days
- Combined with a GHRP (not Ipamorelin specifically in this study): further amplified the GH pulse
- IGF-1 elevation: maintained 20–55% above baseline for the 14-day study period
IGF-1 Elevation from the Stack#
The clinical and research relevance of GH pulsation is mediated primarily through IGF-1 — the downstream anabolic mediator produced mainly in the liver in response to GH.
For the Ipamorelin + CJC-1295 combination, chronic dosing (2–3× daily × 2–4 weeks) produces:
- IGF-1 elevation: 30–60% above baseline (vs. 20–40% for each compound alone)
- Duration: maintained throughout the dosing period, declining to baseline 1–2 weeks after cessation
- Tissue-level IGF-1: local IGF-1 in muscle and bone may be elevated more than serum IGF-1 (local GH→local IGF-1 production in tissue)
The higher IGF-1 elevation from the combination vs. monotherapy is the primary endpoint for body composition research with this stack.
Protocol Design: Timing and Scheduling#
The pulsatile GH rationale#
Pituitary GH secretion is naturally pulsatile. Continuous GH elevation (as from recombinant GH) causes receptor downregulation and desensitization. Pulsatile stimulation — mimicking the natural rhythm — preserves receptor sensitivity.
For CJC-1295 without DAC (short-acting, 30 min t½) + Ipamorelin:
- Administer 2–3× per day, separated by 4–8 hours
- Each injection produces one discrete GH pulse
- Between pulses, GH falls to baseline, preserving receptor sensitivity
For CJC-1295 with DAC (long-acting, 6–8 day t½):
- Administer 1× per week (DAC only)
- Combine with Ipamorelin 2–3× daily for the GHS-R1a component
- The DAC provides continuous GHRH-R "priming"; Ipamorelin pulses provide the GH exocytosis trigger
Timing relative to food/sleep (rodent models)#
GH pulses are naturally largest during fasting and early sleep:
- Fasted state: lower glucose and insulin → less somatostatin inhibition → larger GH pulse
- Sleep onset: the largest GH pulse of the day occurs in the first hour of slow-wave sleep in humans; in nocturnal rodents, at the beginning of the dark cycle
For maximal GH response in rodent research, dose at the onset of the dark cycle (when the animals are becoming active) and consider overnight fast before dosing.
CJC-1295 Without DAC vs. With DAC#
Two distinct versions of CJC-1295 are used in research, with very different pharmacokinetics:
| Parameter | CJC-1295 without DAC (Mod-GRF 1-29) | CJC-1295 with DAC |
|---|---|---|
| Plasma t½ | 30–60 minutes | 6–8 days |
| Dosing frequency | 2–3× daily | 1× weekly |
| GH pulse profile | Sharp, pulsatile | Continuous GHRH stimulation |
| IGF-1 profile | Pulsatile elevation | Sustained elevation |
| Physiological mimicry | Closer to natural GHRH | Non-pulsatile (pharmacological) |
| Research preference | Most research protocols | Less common |
For research attempting to mimic physiological GH pulsation as closely as possible, CJC-1295 without DAC (Mod-GRF 1-29) is the appropriate choice. It is dosed alongside Ipamorelin at the same injection times.
For research where sustained IGF-1 elevation is the endpoint (rather than pulse dynamics), CJC-1295 with DAC simplifies the protocol (once weekly injection vs. 2–3× daily).
Body Composition Research with the Stack#
The Ipamorelin + CJC-1295 combination is most extensively studied in body composition contexts:
Lean mass#
Chronic dosing (2× daily × 4–12 weeks in rodents):
- Increased lean body mass (muscle protein accretion via IGF-1→mTOR→protein synthesis)
- Greater effect than Ipamorelin alone (higher IGF-1 from combination)
- Effect requires adequate dietary protein (IGF-1 drives protein synthesis but requires amino acid substrate)
Fat mass#
GH is directly lipolytic via hormone-sensitive lipase (HSL) activation in adipose tissue. The combination produces greater GH exposure than monotherapy → more visceral and subcutaneous fat mobilization. Published studies in GH-deficient models show 10–20% reduction in fat mass over 8–12 week combination protocols.
Bone#
IGF-1→osteoblast activation produces:
- Increased bone mineral density by DXA
- Greater cortical bone thickness by micro-CT
- Improved bone mineral content
- Effects similar in magnitude to recombinant GH at equivalent IGF-1 elevation levels, but with preserved pulsatile GH pattern that may better maintain osteoblast sensitivity
Frequently Asked Questions#
Do Ipamorelin and CJC-1295 have to be given at the same time? For maximum synergy, yes — they should be co-administered (same injection session, different syringes). The somatostatin-suppressing effect of Ipamorelin is most relevant during the CJC-1295-stimulated GH pulse window. Separating them by hours reduces the synergistic benefit.
Can they be mixed in the same syringe? Many research protocols mix them in the same syringe for SC injection. No published stability incompatibility has been documented for this combination. However, formal co-formulation stability data is limited; preparing separately and administering consecutively is the more conservative approach.
How long before IGF-1 rises? Measurable IGF-1 elevation from the combination is typically detectable within 7 days of starting twice-daily dosing. Peak steady-state elevation is reached in 3–4 weeks.
Is there desensitization to the combination over time? Published studies up to 12 weeks show maintained GH and IGF-1 elevation without documented tachyphylaxis to the combination. The pulsatile nature of the dosing (returns to baseline between injections) is thought to prevent receptor downregulation.
What is the preferred CJC-1295 form for combining with Ipamorelin? Most research uses CJC-1295 without DAC (Mod-GRF 1-29) to maintain pulsatile GH dynamics alongside Ipamorelin pulses. The with-DAC version can be used for sustained IGF-1 elevation protocols.
