The ipamorelin + CJC-1295 combination is the most well-characterized GH secretagogue stack in research literature. The mechanistic basis for their synergy is well-defined: two completely different receptor systems converge on the same somatotroph cells, producing a GH release response that is 2–4× greater than either compound alone. This guide covers the receptor pharmacology, protocol structure, and research applications of the GH stack.
The Receptor Synergy Explained#
Two independent receptors on the same cell:
Pituitary somatotroph cells express both:
- GHRHR (GH Releasing Hormone Receptor): CJC-1295 target → Gαs → adenylyl cyclase → cAMP → PKA → GH gene expression + GH secretion
- GHSR-1a (Growth Hormone Secretagogue Receptor): Ipamorelin target → Gαq → PLCβ → IP3/DAG → PKC + Ca2+ → GH secretion
Why activating both simultaneously is supra-additive:
- cAMP (from GHRHR) and Ca2+ (from GHSR-1a) converge on different exocytosis triggers in the same cell
- PKA (cAMP → PKA): phosphorylates voltage-gated Ca2+ channels → more Ca2+ entry
- PKC (GHSR-1a → DAG → PKC): sensitizes vesicle fusion machinery
- Combined signal: much greater GH vesicle exocytosis than either signaling cascade alone
- Published GH AUC data: combination typically shows 2–4× greater GH release vs. either compound alone
CJC-1295: With DAC vs. Without DAC#
CJC-1295 without DAC (Mod GRF 1-29):
- t½: ~30 minutes
- Requires: twice daily injection for sustained GH/IGF-1 elevation
- Use case: pulsatile GH research protocols; mimics natural GHRH pulses more closely
- Research benefit: each injection produces a discrete GH pulse → studying pulsatility
CJC-1295 with DAC:
- t½: ~7–8 days (Drug Affinity Complex modification → albumin binding → extended half-life)
- Requires: once weekly injection
- Use case: chronic studies where sustained elevated IGF-1 is required without twice-weekly dosing complexity
- Research benefit: steady-state IGF-1 elevation with simpler administration
Research selection: For short-term GH pulse studies → CJC-1295 without DAC. For chronic body composition or somatopause studies → CJC-1295 with DAC.
Research Protocol#
Standard GH stack protocol (rodent model):
| Compound | Dose | Route | Frequency |
|---|---|---|---|
| Ipamorelin | 200–500 μg/kg | SC | Twice daily (q12h) |
| CJC-1295 (no DAC) | 100–300 μg/kg | SC | Twice daily (same injection) |
| CJC-1295 with DAC | 2 mg/kg | SC | Once weekly |
Combining in the same injection: Ipamorelin and CJC-1295 can be mixed in the same syringe before injection — they are both peptide solutions in BAC water and do not interact chemically. This simplifies twice-daily injection protocols.
Reconstitution:
- Ipamorelin 5mg + 2 mL BAC water = 2.5 mg/mL
- CJC-1295 2mg + 1 mL BAC water = 2 mg/mL
- Mix appropriate volumes in syringe just before injection
Research Applications#
Somatopause research: Aged rodents show reduced GHRH pulse amplitude (hypothalamic GHRH neuron decline) + increased somatostatin tone. The GH stack:
- Ipamorelin → bypasses hypothalamic deficit → direct somatotroph GHSR-1a stimulation
- CJC-1295 → provides GHRHR activation despite reduced endogenous GHRH
- Combined → restores near-physiological GH/IGF-1 levels in aged model
Body composition research: Chronic GH stack treatment in HFD-fed or aged rodents → elevated IGF-1 → mTOR activation → lean mass preservation + fat mobilization (via GH's lipolytic effect). Compare to untreated controls with DEXA at study endpoint.
Comparison studies:
- GH stack vs. MK-677 (oral): MK-677 provides continuous GHSR-1a stimulation vs. pulsatile injection. Compare IGF-1 levels, receptor desensitization, and body composition outcomes.
- GH stack vs. recombinant HGH: GH secretagogues stimulate endogenous GH production; rHGH bypasses axis. Compare pulsatility profiles, IGF-1 elevation, and long-term desensitization.
Measuring GH Stack Efficacy in Research#
Primary endpoints:
- Serum IGF-1 (ELISA): best biomarker for chronic GH axis activity; measure at baseline, 4 weeks, 8 weeks
- GH stimulation test: single dose → frequent blood sampling (every 20 min × 3h) → GH AUC
- Body composition (DEXA): lean mass, fat mass, bone density at baseline and study end
- Grip strength: rotarod performance for functional outcomes
